Incidents¶
What this is¶
Incidents records accidents, incidents, and near misses that occur on a project — a privacy-minimal safety register, separate from the routine daily narrative recorded in Site Reports.
Each record represents one independently recorded event — its own date/ time, type, title, description, optional location, and your own assessment of injury and regulatory reportability.
Who can use it¶
Any authenticated user with access to the project — including the Client role, not just Admin/Super Admin — can record incidents.
Where to find it¶
Project → Health & Safety → Incidents.
What you can record¶
- Event type — Accident, Incident, or Near Miss
- Date and time the event occurred
- Title (a short, concise summary)
- Description (the event and site circumstances)
- Location (optional)
- Injury — Not confirmed, No injury, or Injury occurred
- Regulatory reportability — Unknown, Not reportable, or Reportable
- Status — Open or Closed
Privacy by design¶
Incidents deliberately does not record:
- Injured person names or identities
- Worker profiles or employee numbers
- Medical diagnoses, treatment details, or other health information
- Home addresses or dates of birth
There is currently no evidence/attachment capability for incidents — only the fields listed above.
Injury¶
Injury is a genuine three-state field, not a yes/no toggle:
- Not confirmed — the default; injury status has not yet been established
- No injury — explicitly confirmed that no one was injured
- Injury occurred — an injury did occur
"Not confirmed" is never displayed or treated as "No injury."
Regulatory reportability¶
Regulatory reportability (Unknown, Not reportable, Reportable) is always your own manual classification. SureSign does not determine legal reportability under any jurisdiction's regulations, and marking an event "Reportable" does not mean it has actually been reported to a regulator — those remain two separate facts you track yourself.
Status¶
An incident can be Open or Closed, and can be reopened if needed. Closing an incident is a record-keeping status only — it does not represent a legal determination that the matter is fully resolved.
Friday Packs¶
Incidents feeds the Friday Pack's Accidents / Incidents / Near Misses section automatically — every event recorded within a Friday Pack's Monday to Friday reporting week appears there as a short summary (never the fuller description). See Friday Packs.
